PDPV is mainly asymptomatic and associated intestinal anomalies are often more likely to cause high intestinal obstruction. A neonate with recurrent bilious vomiting since birth is presented. Laparotomy revealed PDPV but it was intestinal malrotation that was causing intestinal obstruction.
